This the standard PADI Discover Scuba document, This needs to be completed and handed in to reception on your arrival. If you signing up to do a course the medical section of this must be filled in. If you answer "Yes" to any of the questions then please consult an instructor.

If you're signing up for any of of the courses and have answered yes to any of the questions in the medical statement, one of our instructors will ask you to bring the following 6 page Medical statement to your Doctor to be completed with a full medical.Detailed Medical Form 139k
